1. All requests must be submitted on a Facility Usage Request form. (contact Carol for login info), 10 days prior to the date the facility is needed.
2. All incomplete forms will be returned to the person issuing the request and will not be processed until a completed form has been resubmitted.
3. Request is subject to events already on the calendar.
4. To Enter and Exit the building use the Administrative entrance on the west side of the building.
5. After the event be sure all lights are off (including the bathrooms) and all
doors are locked.
6. You are responsible for your own clean-up unless cleared with Terry Harris. Clean up includes returning the area to the way you found it before you had your event. (See Room Usage Procedures for more details)
7. You are responsible for your own decorating.
8. If you use table cloths, it is your responsibility to wash them and iron them before returning them to the kitchen.
9. If you use the kitchen, you are responsible for clean up of any dishes, utensils, silverware, etc. and need to return them to where you found them.
10. No smoking. No alcoholic beverages.
